REQUIREMENTS FOR REPORT OF MARRIAGE

1. Marriage Contract (obtained from the Marriage Bureau to where the marriage was performed) duly authenticated by:

1. The Local Notary Public Office (with English translation)
2. The Ministry of Foreign Affairs in Beijing

*Please provide at least four (4) copies of the above final original documents.

2. MFA- Authenticated Marriage Contract should likewise be authenticated by The Philippine Embassy in Beijing.

Philippine Authentication Fee –RMB 250

3. Fill out legibly the Report of Marriage form in quadruplicate (4) copies.
4. Four (4) copies of Original Authenticated Marriage Contract
5. Four (4) passport size photos each for the husband and wife
6. Four (4) copies of the data page of their passports.
